The original author has stepped back from developing PHProxy.
Nevertheless, you can still download the script bundled together with the most PHProxy template out there.

Try going to http://www.xytheme.com and choose one of the PHProxy template there, it comes with PHProxy script.

Yepa you just have to search for free phproxy templates in google. All proxy templates should come integrated with phproxy script.
